Advance Health Care Directives

The Advance Health Care Directive is a California document which allows you to choose someone you trust to make decisions about your health care, in the event you become unable to make those decisions for yourself, or if you simply wish to leave those decisions in the hands of another, even if you are still functional.

An Advance Health Care Directive allows you to state in advance what your decisions would be in certain situations, such as the use or removal of life support in the event of a terminal illness or injury. It can also provide instructions for donation of organs if you would choose to do so. The Advance Health Care Directive is where you can state your desires regarding your burial or cremation, funeral or memorial, and provides that your health care agent (or someone else you name) is in charge of those arrangements.

In addition to naming an agent to make health care decisions, you can state who your preferred primary physician is, and give any relevant instructions you may wish to give regarding the management of your health care during times of incapacity.

Knowing your wishes ahead of time gives your loved ones the confidence they need that the end-of-life decisions they may be making for you are the right decisions, based on what you have chosen to do.
